Course Registration

Registration fees are $550 for physicians and $450 for all others. Early registration is encouraged. Registrations received after January 2, 2008 will be charged an additional $50.00 late fee. Refund requests will be honored (less $50.00 processing fee) if we are notified of cancellation, in writing, by January 2, 2009. We reserve the right to cancel this course.
